Sleep & insomnia

Understand your nights. Support your days.

Our proposed sleep pathway brings together a sleep assessment, cognitive behavioural therapy for insomnia and appropriate investigation or referral.

Understand your sleep.

A sleep assessment would explore your sleep pattern, how long difficulties have lasted, daytime tiredness, work and daily routines, mental and physical health, current medicines, alcohol and caffeine. A sleep diary may help build a clearer picture.

Difficulty sleeping can have different causes. Loud snoring, witnessed pauses in breathing or marked daytime sleepiness may need assessment for sleep apnoea rather than treatment for insomnia alone.

CBT for insomnia.
Practical, lasting skills.

Cognitive behavioural therapy for insomnia (CBT-I) is a structured approach that addresses thoughts, habits and routines that keep sleep difficulties going. It goes beyond general sleep tips.

We are developing routes to appropriately qualified support and suitable digital sleep programmes, selected around individual needs. A programme may involve a sleep diary, guided activities and progress reviews.

Digital support.
Options under review.

We are exploring Sleepio and Sleepstation as possible digital sleep-support options. There is no confirmed First Reach partnership, referral agreement or included access with either provider.

Before any programme is offered, its suitability, access route, costs, support and information-sharing arrangements will be explained. Independent enrolment is separate from First Reach care.

When a sleep test
may help.

If symptoms suggest sleep apnoea, a clinician may recommend a sleep service or a home investigation where clinically appropriate. Home testing is not suitable for everyone and does not assess every cause of poor sleep.

We are exploring an arrangement with Intus Healthcare for suitable sleep-apnoea investigations. No First Reach testing or referral arrangement is currently confirmed. The proposed pathway would cover suitability, consent, the test report, clinical interpretation and onward care if needed.

A plan that
can be reviewed.

The proposed pathway includes agreed goals, a review of sleep and daytime functioning, and support to adjust the plan if it is not helping. Review timing and contact arrangements will be confirmed before booking opens.

Any medicine would require an individual clinical decision under an approved protocol, including risks, interactions and monitoring. Specific medicines are not offered or guaranteed on this page.

Clear responsibility.
Personal decisions.

Smiths Medical Group Ltd is responsible for clinical assessment, prescribing decisions and clinical governance. First Reach Digital Ltd manages the website and non-clinical administration. Any prescribed medicines are subject to independent checks by Jayty Ltd through Inspire Pharmacy — Ibstock.

An appointment does not guarantee treatment or a prescription. If online care is unsuitable, further examination, investigation or another service may be needed.
